Contraindications

  • Ophthalmic solution: Known hypersensitivity to olopatadine or any ingredient in the formulation.1

Nasal spray: Manufacturer states none known.17

Warnings/Precautions

Proper Handling to Avoid Contamination of Ophthalmic Solution

To avoid contamination of the dropper tip and solution, do not to touch the eye, eyelids, or surrounding areas with the dropper tip.1 12 Close container tightly when not in use.1

Contact Lenses

Ophthalmic solution not indicated for contact lens-related ocular irritation.1 16 Do not wear contact lenses if eyes are red.1

Remove soft contact lenses prior to administration of each ophthalmic dose, since benzalkonium chloride preservative may be absorbed by soft contact lenses;12 16 lenses may be reinserted after ≥10 minutes following administration.1 16

Nasal Effects

Epistaxis, nasal ulcerations, and nasal septal perforations reported with nasal spray.17 Perform nasal examination prior to initiating intranasal olopatadine therapy to ensure absence of nasal diseases (other than allergic rhinitis).17 Perform nasal examinations periodically for signs of adverse effects on the nasal mucosa.17 If nasal ulcerations occur, consider discontinuing nasal spray.17

CNS Effects

Somnolence reported with nasal spray.17 Avoid engaging in activities requiring complete mental alertness and motor coordination (e.g., driving or operating machinery) after administration of nasal spray.17 Concomitant use of nasal spray with alcohol or other CNS depressants may result in additive CNS depression; therefore, avoid such concomitant use.17

Specific Populations

Pregnancy

Category C.1

Lactation

Distributed into milk in rats following oral administration.1

Ophthalmic solution: Not known whether distributed into human milk following topical application to the eye.1 Use with caution.1

Nasal spray: Use only if potential benefits outweigh risks.17

Pediatric Use

0.1% ophthalmic solution: Safety and efficacy not established in children <3 years of age.1 Appears to be well-tolerated in children 3–16 years of age.11 13

0.2% ophthalmic solution : Safety and efficacy not established in children <2 years of age.16

Nasal spray: Safety and efficacy not established in children <6 years of age.17 In clinical studies, incidence of epistaxis was higher in children 6–11 years of age (5.7%) compared with adolescents or adults (3.2%).17

Geriatric Use

Ophthalmic solution: No substantial differences in safety and efficacy relative to younger adults.1 16

Nasal spray: Insufficient experience in patients ≥65 years of age to determine whether geriatric patients respond differently than younger adults.17 Other clinical experience has not identified differences in responses between geriatric and younger patients.17 Select dosage with caution because of age-related decreases in hepatic, renal, or cardiac function and potential for concomitant disease and drug therapy.17

Common Adverse Effects

0.2% ophthalmic solution: Symptoms similar to cold syndrome, pharyngitis.16

Nasal spray: In adults and children ≥12 years of age, bitter taste, headache, epistaxis, pharyngolaryngeal pain, postnasal drip, cough, urinary tract infection.17 In children 6–11 years of age, epistaxis, headache, upper respiratory tract infection, bitter taste, pyrexia, rash.17
