Therapeutic Class: Antitussive, Opioid/Decongestant Combination

Chemical Class: Hydrocodone
Uses For Pancof HC
Hydrocodone and pseudoephedrine combination is used in adults to relieve cough and nasal congestion (stuffy nose) caused by the common cold.
Hydrocodone is a narcotic antitussive (cough suppressant). It acts directly on the cough center in the brain to relieve cough. Pseudoephedrine is a decongestant, which decreases nasal congestion by narrowing the blood vessels and reducing blood flow to the nasal passage.
When hydrocodone is used for a long time, it may become habit-forming, causing mental or physical dependence. However, people who have continuing cough and nasal congestion should not let the fear of dependence keep them from using narcotics to relieve their cough. Mental dependence (addiction) is not likely to occur when narcotics are used for this purpose. Physical dependence may lead to withdrawal side effects if treatment is stopped suddenly. However, severe withdrawal side effects can usually be prevented by gradually reducing the dose over a period of time before treatment is stopped completely.
This medicine is available only with your doctor's prescription.
Before Using Pancof HC
In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For this medicine, the following should be considered:
Allergies
Tell your doctor if you have ever had any unusual or allergic reaction to this medicine or any other medicines. Also tell your health care professional if you have any other types of allergies, such as to foods, dyes, preservatives, or animals. For non-prescription products, read the label or package ingredients carefully.
Pediatric
Use of hydrocodone and pseudoephedrine combination is not indicated in the pediatric population. Safety and efficacy have not been established.
Geriatric
Appropriate studies performed to date have not demonstrated geriatric-specific problems that would limit the usefulness of hydrocodone and pseudoephedrine combination in the elderly. However, elderly patients are more likely to have age-related kidney problems, which may require caution and an adjustment in the dose for patients receiving this medicine.

For Healthcare Professionals
Applies to hydrocodone / pseudoephedrine: oral liquid, oral liquid extended release, oral syrup, oral tablet
Nervous system
Opioids may result in psychotic symptoms in some patients.[Ref]
Nervous system side effects of hydrocodone have included mental and respiratory depression, stupor, delirium, somnolence, and dysphoria. Nervous system side effects of pseudoephedrine have included insomnia in up to 30% of patients. Tremor, anxiety, nervousness, and headache have also been reported.[Ref]
Other
Other side effects of hydrocodone have included withdrawal symptoms, such as agitation, restlessness, anxiety, insomnia, tremor, abdominal cramps, blurred vision, vomiting and sweating, after either abrupt cessation or fast tapering of narcotic analgesics.[Ref]
Gastrointestinal
Gastrointestinal side effects of hydrocodone have included nausea, vomiting, constipation, and dry mouth. Gastrointestinal side effects of pseudoephedrine have included anorexia and gastric irritation in approximately 5% of patients. Dry mouth, nose, or throat have occurred in up to 15% of patients.[Ref]
Cardiovascular
Cardiovascular side effects of pseudoephedrine have included tachycardia. Some patients have developed hypertension and/or arrhythmias.[Ref]
Pseudoephedrine causes vasoconstriction which generally does not produce hypertension, but may be problematic for patients with preexisting hypertension. Arrhythmias may be produced in predisposed patients. Rarely, pseudoephedrine has been reported to cause coronary artery spasm and chest pain.
One report evaluated the effect with 60 mg of pseudoephedrine on individuals in a hyperbaric chamber at 1 atmosphere (simulated scuba dive to 66 feet of sea water). Pseudoephedrine and depth (simulated) were found to have significant but opposite effects on heart rate, although these effects were unlikely to be clinically significant during diving.[Ref]
Genitourinary
Genitourinary side effects of hydrocodone have included ureteral spasm, spasm of vesicle sphincters, and urinary retention.[Ref]
Dermatologic
Dermatologic side effects of hydrocodone have included rash.[Ref]
