What should I discuss with my healthcare provider before taking Alahist DM (brompheniramine, dextromethorphan, and phenylephrine)?

Do not use this medication if you have used an MAO inhibitor such as furazolidone (Furoxone), isocarboxazid (Marplan), phenelzine (Nardil), rasagiline (Azilect), selegiline (Eldepryl, Emsam, Zelapar), or tranylcypromine (Parnate) in the last 14 days. A dangerous drug interaction could occur, leading to serious side effects.

You should not use brompheniramine, dextromethorphan, and phenylephrine if you have severe constipation, a blockage in your stomach or intestines, or if you are unable to urinate.

Do not use this medication if you have untreated or uncontrolled diseases such as glaucoma, asthma or COPD, high blood pressure, heart disease, coronary artery disease, or overactive thyroid.

Ask a doctor or pharmacist if it is safe for you to take this medication if you have:

  • a colostomy or ileostomy;

  • diabetes;

  • liver or kidney disease;

  • epilepsy or other seizure disorder;

  • cough with mucus, or cough caused by emphysema or chronic bronchitis;

  • enlarged prostate or urination problems;

  • low blood pressure;

  • pheochromocytoma (an adrenal gland tumor); or

  • if you take potassium (Cytra, Epiklor, K-Lyte, K-Phos, Kaon, Klor-Con, Polycitra, Urocit-K).

FDA pregnancy category C. It is not known whether this medication will harm an unborn baby. Do not use this medicine without medical advice if you are pregnant.

This medicine may pass into breast milk and may harm a nursing baby. Antihistamines and decongestants may also slow breast milk production. Do not use this medicine without medical advice if you are breast-feeding a baby.

What happens if I miss a dose?

Since this medication is taken when needed, you may not be on a dosing schedule. If you are taking the medication regularly, take the missed dose as soon as you remember. Skip the missed dose if it is almost time for your next scheduled dose. Do not take extra medicine to make up the missed dose.

What are some side effects that I need to call my doctor about right away?

WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical help right away if you have any of the following signs or symptoms that may be related to a very bad side effect:

  • Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.

Cardiovascular

Cardiovascular side effects have included hypotension, hypertension, cardiac arrhythmias, palpitations, tachycardia and pallor.[Ref]

Dermatologic

Dermatologic side effects have included urticaria, drug rash, photosensitivity, and pruritus.[Ref]

Nervous system

Nervous system side effects have included disturbed coordination, tremor, irritability, insomnia, weakness, nervousness, convulsions, headache, euphoria, dysphoria, restlessness, hallucinations, and central nervous system depression.[Ref]

Ocular

Ocular side effects have included visual disturbances.[Ref]

Respiratory

Respiratory side effects have included thickening of bronchial secretions, respiratory difficulty, shortness of breath, tightness of chest and wheezing.[Ref]

Gastrointestinal

Gastrointestinal side effects have included dryness of mouth, nose and throat, epigastric discomfort, nausea, vomiting, diarrhea, and constipation.[Ref]

Genitourinary

Genitourinary side effects have included urinary frequency and difficult urination.[Ref]

Hematologic

Hematologic side effects have included hemolytic anemia, thrombocytopenia, and agranulocytosis.[Ref]

Usual Adult Dose for Cough and Nasal Congestion

Brompheniramine/DM/Phenylephrine 2 mg-15 mg-7.5 mg/5 mL oral liquid:
10 mL orally every 6 hours not to exceed 40 mL daily.

Brompheniramine/DM/Phenylephrine 1 mg-5 mg-2.5 mg/5 mL oral liquid:
20 mL orally every 4 to 6 hours not to exceed 120 mL daily.

Brompheniramine/DM/Phenylephrine 4 mg-15 mg-7.5 mg/5 mL oral liquid:
5 mL orally every 4 to 6 hours not to exceed 30 mL daily.

Brompheniramine/DM/Phenylephrine 8 mg-20 mg-20 mg/5 mL oral suspension, extended release:
10 mL orally every 12 hours not to exceed 20 mL daily.

Brompheniramine/DM/Phenylephrine 10 mg-30 mg-25 mg/5 mL oral suspension, extended release:
10 mL orally every 12 hours not to exceed 20 mL daily.

Brompheniramine/DM/Phenylephrine 2 mg-10 mg-5 mg/5 mL oral liquid:
10 mL orally every 4 hours not to exceed 60 mL daily.

Brompheniramine/DM/Phenylephrine 4 mg-20 mg-10 mg/5 mL oral liquid:
5 to 10 mL orally every 4 to 6 hours not to exceed 40 mL daily.
