What is Allfen CD (codeine and guaifenesin)?
Codeine is a narcotic cough suppressant. It affects the signals in the brain that trigger cough reflex.
Guaifenesin is an expectorant. It helps loosen congestion in your chest and throat, making it easier to cough out through your mouth.
Codeine and guaifenesin is a combination medicine used to treat cough and chest congestion caused by allergies, the common cold, or the flu.
This medicine will not treat a cough that is caused by smoking, asthma, or emphysema.
Codeine and guaifenesin may also be used for purposes not listed in this medication guide.
What is the most important information I should know about Allfen CD (codeine and guaifenesin)?
Codeine can slow or stop your breathing, and may be habit-forming. MISUSE OF THIS MEDICINE CAN CAUSE ADDICTION, OVERDOSE, OR DEATH, especially in a child or other person using the medicine without a prescription.
Ask a doctor before giving this medicine to a child younger than 12 years old. Do not give this medicine to anyone under 18 who recently had surgery to remove the tonsils or adenoids.
What should I avoid while taking Allfen CD (codeine and guaifenesin)?
This medicine may impair your thinking or reactions. Avoid driving or operating machinery until you know how this medicine will affect you. Dizziness or severe drowsiness can cause falls or other accidents.
Do not drink alcohol. Dangerous side effects or death could occur.
Ask a doctor or pharmacist before using any other cough or cold medicine. Many combination medicines contain guaifensin. Taking certain products together can cause you to get too much of this medicine.
What other drugs will affect Allfen CD (codeine and guaifenesin)?
Taking codeine and guaifenesin with other drugs that make you sleepy or slow your breathing can cause dangerous side effects or death. Ask your doctor before taking a sleeping pill, narcotic pain medicine, prescription cough medicine, a muscle relaxer, or medicine for anxiety, depression, or seizures.
Other drugs may interact with codeine and guaifenesin, including prescription and over-the-counter medicines, vitamins, and herbal products. Tell your doctor about all your current medicines and any medicine you start or stop using.

Allfen CD Side Effects
Along with its needed effects, a medicine may cause some unwanted effects. Although not all of these side effects may occur, if they do occur they may need medical attention.
Although serious side effects occur rarely when this medicine is taken as recommended, they may be more likely to occur if: too much medicine is taken, it is taken in large doses, or it is taken for a long period of time.
Get emergency help immediately if any of the following symptoms of overdose occur:

- Cold, clammy skin

- shortness of breath or troubled breathing
- slow or fast heartbeat

- increased watering of mouth
- loss of appetite
- metallic taste
- skin rash, hives, or redness
- sore throat
- swelling of face, lips, or eyelids
- Unexplained sore throat and fever
- unusual tiredness or weakness
- yellow eyes or skin
Some side effects may occur that usually do not need medical attention. These side effects may go away during treatment as your body adjusts to the medicine. Also, your health care professional may be able to tell you about ways to prevent or reduce some of these side effects. Check with your health care professional if any of the following side effects continue or are bothersome or if you have any questions about them:
- Constipation
- decreased sweating
- difficult or painful urination
- dizziness or lightheadedness
- drowsiness
- dryness of mouth, nose, or throat
- false sense of well-being
- increased sensitivity of skin to sun
- nausea or vomiting
- nightmares
- stomach pain
- thickening of mucus
- trouble in sleeping
- unusual excitement, nervousness, restlessness, or irritability
- unusual tiredness or weakness
Not all of the side effects listed above have been reported for each of these medicines, but they have been reported for at least one of them. There are some similarities among these combination medicines, so many of the above side effects may occur with any of these medicines.

What are some side effects that I need to call my doctor about right away?
WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical help right away if you have any of the following signs or symptoms that may be related to a very bad side effect:
- Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.
- Very bad dizziness or passing out.
- Chest pain or pressure or a fast heartbeat.
- Trouble breathing, slow breathing, or shallow breathing.
- Noisy breathing.
- Feeling very sleepy.
- Feeling confused.
- A heartbeat that does not feel normal.
- Hallucinations (seeing or hearing things that are not there).
- Mood changes.
- Seizures.
- Very bad belly pain.
- Very bad headache.
- Trouble passing urine.
- Shakiness.
- Change in eyesight.
- Very upset stomach or throwing up.
- Very hard stools (constipation).

Respiratory depression and death have occurred in children who received codeine in the postoperative period following tonsillectomy and/or adenoidectomy. Further investigation has shown these patients to be ultra-rapid metabolizers of codeine. Children with obstructive sleep apnea who are treated with codeine appear to be particularly sensitive to the respiratory depressant effects of codeine.[Ref]

Usual Pediatric Dose for Cough
Less than 12 years of age: Not Recommended
12 years or older:
1 dose orally every 4 to 6 hours as needed
-Codeine: Single dose: up to 20 mg; Maximum dose: 120 mg in 24 hours
-Guaifenesin: Single dose: up to 400 mg; Maximum dose: 2400 mg in 24 hours
Maximum dose should not exceed maximum dose of either of the single ingredient doses
Comments:
-Liquid preparations should be measured with an accurate milliliter measuring device.
-Codeine should not be used in adolescents who are obese or have conditions such as sleep apnea or severe lung disease which may increase the risk of serious breathing problems.
Uses: Temporary relief of coughs due to minor throat and bronchial irritations as may occur with the common cold or inhaled irritants; helps loosen phlegm and thin bronchial secretions to make cough more productive.
