What Is Caffeine?

Caffeine citrate is a central nervous system stimulant.

Caffeine citrate is used to treat breathing problems in premature infants.

Caffeine citrate may also be used for purposes not listed in this medication guide.

Caffeine citrate should not be given to a child who has ever had an allergic reaction to this medicine.

Caffeine citrate should not be given to a child who has ever had an allergic reaction to this medicine.

To make sure caffeine citrate is safe for your child, tell your doctor if the child has:

  • seizures;
  • heart disease; or
  • liver or kidney disease.

Uses For caffeine

Caffeine tablets are used as an alertness aid to help you keep awake when you experience drowsiness or unusual tiredness or weakness. Do not use caffeine as substitute for sleep.

Caffeine oral solution is used to treat short-term apnea of prematurity when premature babies (infants between 28 and 32 weeks gestational age) stop breathing. Apnea of prematurity is caused by the baby's breathing centers not being fully developed.

Contraindications

Hypersensitivity to caffeine or any component of the formulation; sodium benzoate is not for use in neonates

Dosing Renal Impairment

There are no dosage adjustments provided in the manufacturer's labeling. Use with caution.

Pregnancy Risk Factor C Pregnancy Considerations

Adverse events were observed in animal reproduction studies. Caffeine crosses the placenta; serum concentrations in the fetus are similar to those in the mother (Grosso 2005). Based on current studies, usual dietary exposure to caffeine is unlikely to cause congenital malformations (Brent 2011). However, available data shows conflicting results related to maternal caffeine use and the risk of other adverse events, such as spontaneous abortion or growth retardation (Brent 2011; Jahanfar 2013). The half-life of caffeine is prolonged during the second and third trimesters of pregnancy and maternal and fetal exposure is also influenced by maternal smoking or drinking (Brent 2011; Koren 2000). Current guidelines recommend limiting caffeine intake from all sources to ≤200 mg/day (ACOG 2010).

Usual Pediatric Dose for Apnea of Prematurity

Caffeine Citrate:
28 Weeks to Less Than 33 Weeks Gestational Age:
-Loading Dose: 20 mg/kg IV over 30 minutes one time.
-Maintenance Dose: 5 mg/kg orally or IV over 10 minutes every 24 hours.
-Treatment Duration: Safety and efficacy of this drug for treatment of apnea of prematurity have not been established beyond 12 days.

Use: Short-term treatment of apnea of prematurity in infants.

Renal Dose Adjustments

Caffeine Citrate: Use with caution in infants with impaired renal function; monitor serum drug concentrations and adjust dose accordingly to avoid toxicity.

Liver Dose Adjustments

Caffeine Citrate: Use with caution in infants with impaired hepatic function; monitor serum drug concentrations and adjust dose accordingly to avoid toxicity.
